Client Bill of Rights:

Melanie Barnum is a personal healer and has received training and certifications in many healing modalities.  Melanie is a psychic intuitive, medium, intuitive counselor and author. She specializes in providing in depth readings which cover a wide range of personal information.  These may include situations, circumstances, or people from the past, the present, and the future.  Melanie also connects to loved ones (and sometimes not so loved ones) on the other side, or who have passed on.  Though she provides no guarantee that the person you’d like to connect to will come through Melanie does offer a plethora of intuitive wisdom to aid you in moving forward in your life through her contact with the other side and her psychic gifts.  She asks only that you be open.

Confidentiality:  I will not release any specific information about you to anyone without a written authorization from you, except as provided by law.   I will retain the right to use or reference your sessions as case studies, for education or literary purposes.  You have a right to be allowed access to any written record about you.

Fees:  My hourly fee is $250.  If more time is needed, this will be charged accordingly.  Payment is due in full per session unless paid in advance.  If you need to reschedule or cancel any future appointment please do so 24 hours in advance in order to avoid charging you for the time reserved.  All fees paid are non-refundable. Bounced check fees of $25 per occurrence will be charged if necessary.

Client Statement of Informed Consent

I have read the above statement and wish to participate in this education relationship with Melanie Barnum.  I seek the opportunity to consider her advice, recognizing I am free to act upon or disregard her recommendations as I choose, and as such, release her all responsibility for my actions and any consequences thereof, both now and in the future.  I understand the recommendations are educational and for entertainment purposes only, and no guarantees are made.
